Selecting the primary patient care home service provider
Urgent for you. This is called your primary care provider. People Home Care is the support of all the nurses in your home who manage your overall healthcare needs. They provide services such as monthly checkups, and treatment of illness and injury. This service includes practicing nurses, general nurses, internist nurses. If you need help, your capacity care coordinator can help you with your home.
To select or change your nurse, you can call the empowered member services at 01712544008 or contact your care coordinator directly. Both Medicare and Medicaid members can keep their existing Medicare if they wish.

Call your care coordinator first if you need care. You can reach Care Adjustment 24/7. Your care coordinator will help you manage all your health services. If you think you need to see a specialist or another provider, talk to your nurse. Your nurse can help you decide if you need to see another provider.
It is very important that you keep all your appointments for a doctor’s visit, lab test, or X-ray. If you are unable to make an appointment, please call your nurse at least one day in advance. If you need help making an appointment, please contact your care coordinator directly or empower member services 01712544008.

Emergency care
Immediate care A health problem requires care right now but not life threatening. Emergency care is not emergency care. If you go to an emergency care clinic without an empowerment network, you will need prior approval from Empowerment. Here are some examples of emergency care:

Emergency services
An emergency medical condition is extremely serious. It can even be fatal. An emergency is suddenly serious and puts you at risk of harm. You may have severe pain, injury or illness. This can happen when your body is not working as it should. Call immediately if you have an emergency.

Services
Empowerment covers all medically necessary Medicaid services. We cover the services at no cost to you. Some services require prior approval from the power requirement Your health care provider will submit any required prior approval. We do not need to contact you about this. Empowerment will not deny services due to moral or religious issues.
